The plan operates on an annual membership basis at approximately $600 per year, which can be paid in monthly installments of $50 for budget convenience. This predictable cost covers all your maintenance needs and warranty protection for the membership year.
Members schedule regular maintenance appointments—typically twice yearly—for professional cleaning and evaluation of implant health. These appointments are included in your membership and ensure we monitor your implants consistently throughout the warranty period.

Issues arising from missed maintenance appointments, inadequate home care, smoking, or failure to follow clinical recommendations are excluded from warranty coverage. The warranty requires your active participation in maintaining your implant health through consistent care and healthy choices.
Complications related to pre-existing health conditions that were disclosed during treatment planning but develop or worsen after implant placement may not be covered. Warranty coverage begins upon completion of your implant treatment and final restoration placement. The five-year warranty period starts from this completion date, and your membership should begin immediately to ensure continuous coverage throughout the warranty period.
Missing scheduled maintenance appointments may void your warranty coverage. However, we understand that scheduling conflicts occur. Contact our office immediately if you need to reschedule a maintenance appointment, and we’ll work to find an alternative date that maintains your warranty eligibility.
You may cancel your membership at any time, but doing so terminates your warranty coverage. Warranty protection requires continuous membership and consistent maintenance attendance. If you cancel and later wish to re-enroll, coverage terms may be different based on your implant status and time elapsed since treatment.
The Implant Membership Plan covers only implants placed by Dr. Long at our practice. We cannot provide warranty coverage for implants placed elsewhere, as we have no control over the planning, placement, or initial care of those implants.
If you relocate and can no longer attend our practice for maintenance, we can provide your complete implant records to your new dentist. However, warranty coverage terminates when you transfer care to another provider, as we can only guarantee work we directly monitor and maintain.
The warranty covers functional and health-related complications, not aesthetic preferences that develop after you’ve approved and accepted your restoration. If you become dissatisfied with the appearance of your restoration for reasons unrelated to defects or damage, changes would be considered elective and not covered under warranty terms.
